A. I hereby knowingly and freely assume all of the risks, both known and unknown, of participating in Edu-Futuro's Soccer Tournament (forwardly known as "Edu-Futuro Cup"), either as Player or Volunteer, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them, or because of their possible liability without fault.

B. I consent and agree that Edu-Futuro, and/or their coaches, agents, representatives or volunteers may take photographs or digital recordings of me as a participant during this event and use these in any and all media for training or promotional purposes. I further consent that my identity may be revealed therein or by description text or commentary. I waive any rights, claims or interest and I understand that there will be no financial or other remuneration. The accident waiver, release of liability and image release shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

C. I understand that it is my responsibility to show respect and sportsmanship to every tournament participant, volunteer, medical staff, Edu-Futuro staff, Associate Council Members, Board of Directors, and Arlington County Park personnel.

D. I am aware that participating in The Soccer Tournament could be detrimental to my health, especially if I do not already regularly exercise. I certify that I am physically fit and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in The Soccer Tournament. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the organizers of The Soccer Tournament in which I may participate and that it will govern my actions and responsibilities at The Soccer Tournament.

E. I agree to WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, disability, personal injury, property damage, property theft, for my death or actions of any kind which may hereafter occur to me including my traveling to and from The Soccer Tournament THE FOLLOWING ENTITIES OR PERSONS: Edu-Futuro, Arlington County Parks, and/or their coaches, agents, representatives or volunteers.

F. I understand that in order to complete registration I must pay the registry fee online. I will not be allowed to pay the registration fee on the date of The Soccer Tournament. I understand that if I do not pay I will not be allowed to play in the tournament.                  

I further understand and agree to pay the full amount of the registration fee in order to participate in the Soccer Tournament.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT VIA ELECTRONIC FORMAT ON MY OWN FREE WILL.

A. Renuncio a cualquier derecho, reclamo o interés y entiendo que no habrá ninguna remuneración financiera o de otro tipo. E. Acepto RENUNCIAR, LIBERAR Y DESCARGAR de toda responsabilidad, incluida, entre otras, la responsabilidad que surja de la negligencia o culpa de las entidades o personas liberadas, discapacidad, lesiones personales, daños a la propiedad, robo, por mi muerte o acciones de cualquier tipo que puedan ocurrirme en el futuro, incluido mi viaje hacia y desde el Torneo de Fútbol. F. Entiendo que para completar el registro debo pagar la tarifa de registro en línea. No se me permitirá pagar la cuota de inscripción en la fecha del Torneo de Fútbol. Entiendo que si no pago no se me permitirá jugar en el torneo.

Además, entiendo y acepto pagar el monto total de la tarifa de inscripción para participar en el Torneo de Fútbol.

CERTIFICO QUE HE LEÍDO ESTE DOCUMENTO Y ENTIENDO COMPLETAMENTE SU CONTENIDO. SOY CONSCIENTE DE QUE ESTO ES UNA LIBERACIÓN DE RESPONSABILIDAD Y UN CONTRATO Y LO FIRMO MEDIANTE FORMATO ELECTRÓNICO POR MI PROPIA VOLUNTAD.
